Police say they were overwhelmed by the large crowd when a stampede killed four people at a church in Ghana's capital, Accra, on Sunday.
"All of us were caught by surprise. No-one knew the crowd will be so huge," said police spokesman Freeman Tetteh.

The church is run by prominent Nigerian preacher TB Joshua.
He is one of the wealthiest and most popular evangelical clerics in Africa and his Synagogue Church of All Nations claims it can perform miracles, such as curing blindness.
'Devastated'
Mr Tetteh told the BBC's Newsday programme there was a larger than expected crowd at the service after church officials announced that "holy water" would be given to worshippers.
"The church was also caught by surprise... Nobody can apportion blame [at this stage]," he said.
"We need to investigate and establish responsibility."
Some of the wounded were in a critical condition in hospital, Mr Tetteh said.
The stampede took place when people at the back of the church began to push to get to the altar, AFP news agency reports.

"The crowd surged forward and became uncontrollable," Mr Tetteh told the BBC.
One worshipper, Gertrude Sumbamala said she had suffered a broken leg after being caught up in the stampede, AFP reports.
"People from the back, they pushed... and they fell on us. So we were battered by a lot of forces," Ms Sumbamala is quoted as saying.
The church's Rev Sam McCaanan told local radio station Citi News that the stampede was regrettable.
"We are devastated, it's very unfortunate and we are very sorry," he is quoted as saying.

Sadly, all the hysteria about the new, upgraded anointed water packaging turned to tragedy on Sunday when a stampede happened at SCOAN Ghana. 4 people were killed and many others injured, according to a police officer at the scene “15 people who were seriously injured, some with broken limbs, many others collapsed but were resuscitated.” (source)
This crush happened because desperate people sincerely believed that the solution to their problems lay in a little bottle of water. Why do they believe this? Because TB Joshua has told them. In a videoaccompanying the release of the new packaging TB Joshua claims that the water is the solution to your business problems, marital issues, financial constraints, sicknesses and other such issues.
Is there a biblical basis for these claims?
Throughout the Scriptures, we see clear proof that God can use any medium to express Himself:
- In Acts 19:11-12, God used the medium of Paul’s handkerchief and aprons to heal the sick.
- In Exodus 14:16, God used the medium of Moses’ staff to split the Red Sea.
- In Acts 3:6, God used the voice of His servants, Peter and John to raise a crippled man.
- In 2 Kings 5:14, God used the medium of a dirty river to heal Namaan.
- In Acts 2:1-2, God expressed Himself through the medium of sound at the time of Pentecost
- In 1 Samuel 17:49, God used David’s catapult to defeat the giant Goliath.
- In John 9:6-7, Jesus used mud and saliva to heal a blind man.
- In Acts 4:15-16, God used the medium of Peter’s shadow to heal the sick.
This anointed water is billed as TB Joshua’s anointed water. People believe it is through this medium their breakthrough will come because the water “connects” them to TB Joshua and his anointing. This is has nothing to do with the Christian faith. Christians believe that all mankind is fallen and needs redemption through the blood of Jesus, but that redemption is available to all directly – no man of God, priest or intercessor required.
Perhaps, like Peters miraculous escape from prison God will also miraculously grant you breakthrough in an area of suffering, or maybe like James (who was execulted) your breakthrough will come beyond the grave. Maybe your healing will come instantly, or maybe like the great Apostle Paul you will be tormented in your weakness, but through it you will learn that His grace is sufficient for you, His power is made perfect in weakness (2 Cor 12:9).
Four people are dead because they desperately wanted this anointed water. Many others are alleged to have died after naively putting their trust in a little bottle of tap water instead of the medication trained doctors had prescribed.
Don’t put your hope in this water, don’t put your hope in a man who sets himself up as a false christ. We encourage you to pray for those who have lost loved ones, and pray that this deception ends swiftly.
If you’re still not convinced, ask yourself the following questions:
1. If TB Joshua is a prophet with the track record of predicting disasters that he claims, why didn’t he predict this one?
2. If the anointed water has miraculous healing powers, why did the SCOAN branch in Ghana not bring it out to heal those injured, or to raise the dead back to life?

Four persons have been confirmed dead at the Spintex Road branch of the Synagogue Church of All Nations (SCOAN) founded by Prophet T.B. Joshua.
The tragic incident occurred after a stampede ensued at the church Sunday during the distribution of free holy water. The man of God, Prophet TB Joshua had earlier given the announcement on their station, Emmanuel TV, that there was going to be a distribution of anointed water at the service. The holy water, to many TB Joshua followers is acclaimed to be the panacea to the many spiritual afflictions.
Hundreds were said to have started arriving at the church premises from 8pm last night and before the service could start this morning, the stampede occurred because of the overflow. The bodies of the deceased have been deposited at the 37 and Police Hospital mortuaries. Earlier in the morning, a flotilla of cars on the Spintex road and its enclave, en route for an anointing water service by Nigeria’s Prophet T. B. Joshua in Accra, resulted in another impenetrable gridlock which caused frustration to commuters and Residents in that area.
Hundreds of cars belonging to Nigerians in Ghana as well as Ghanaians, were parked on the sides of the road in a mesh of chaos, which spewed onto the roads, thus resulting in a go slow.
Three weeks ago, a similar situation left the Spintex road in another lock down as hundreds of people thronged the church to meet Prophet TB Joshua The Church is yet to formally issue a statement on the incident.
Ghana Police Bans TB Joshua Church From Holding Crusades At Accra Spintex Road

Speaking to Citi News, DCOP Yehonu noted that the stampede that occured at the weekend could have been avoided if the church had chosen a bigger venue to hold its events.
Four people died on Sunday as a result of a stampede at the Synagogue Church of all Nations. The stampede occurred as people struggled to secure poll position to receive holy water blessed by the Presiding Prophet, T.B Joshua.
Mr Tettey Yehonu indicated that, "the place was too small to manage a congregation of this magnitude", and suggested that the Accra Sports Stadium or the independence square were more suitable alternatives to handle a crowd of that magnitude.
Meanwhile, officials of the Church who say this is the first time people have died in any of their services worldwide, have told Citi News they did all in their power to save the situation.
Reverend Sam Mc-Canan is a Reverend at the Synagogue Church of all Nations in Accra said: "The happening yesterday at Spintex is unprecedented, we are used to crowds and huge numbers but not something like this. We are a crowd puller but we are not used to a number like this. This is a first of this kind and we are very devastated about it".
"This is very unfortunate and we are very devastated about it. We are going to do a thorough work to ensure this does not repeat itself. Everything started orderly this morning, until 30 minutes when we heard a stampede outside the office".
"We went outside then saw some casualties lying outside there and others trampled over. We cancelled the meeting and called the national ambulance to intervene".
